Terms and Conditions

INSURANCE:  I understand it is my responsibility to provide for my own (and other members of my family if applicable) accident and health coverage while participating in all YMCA activities. 

MEDICAL RELEASE:  I authorize the YMCA of Southern Nevada to provide or obtain emergency medical attention for me or members of my family in the event of sickness or injury.  I realize and understand that my family insurance policy will be responsible for any accident or medical claim.  Should I or any member of my family, require special medical treatment, prescriptions, or hospital care, I am responsible for all expenses.

PARENTAL CONSENT:  I, the parent or guardian of the child or children included in this membership, do hereby consent that my child has permission to participate as a member of the YMCA of Southern Nevada.  It is understood that while my child is in the YMCA facility that every reasonable precaution will be taken to insure for the supervision of my child.  I will not leave my child ages 8-12 at the YMCA for any longer time than a three (3) hour period on any one day (unless parent is a member and child accompanies adult).

LIABILITY WAIVER: INDEMNIFICATION AND HOLD HARMLESS AGREEMENT

I understand that all exercises may not be suitable for me, and that participation in any exercise program may result in injury and even death.  To my knowledge, I am not infected with a contagious disease or limiting physical condition or disability, especially with respect to my heart that would preclude any exercise program.  I undertake this program at my own risk.

I understand that any instructions or advice presented by the YMCA or members of the YMCA staff are in no way intended to be a substitute for professional medical counseling.  I authorize emergency medical treatment if it becomes necessary.

The YMCA of Southern Nevada will not assume responsibility for any injury incurred while participating in any athletic events, childcare programs, parent/child events and outings, special events, sports and aquatic programs, or any related YMCA sponsored activities. Certain risks of injury are inherent during participation in these programs and events.  Nor will the YMCA of Southern Nevada be responsible for any lost or stolen items while members and/or program participants are using YMCA facilities, on YMCA premises, or on off-site YMCA program locations.  I, the undersigned for myself and my heirs, do hereby release the YMCA of Southern Nevada and its employees and agents from any and all claim for injury, loss, or damage I may suffer as a result of my participation, including any injury caused by negligence, if any, of the YMCA, its officers, employees, agents, volunteers, or the negligence of anyone else.  I give my permission to the YMCA of Southern Nevada to use photographs, film footage, or tape recordings, which may include my image or voice for purposes of promoting or interpreting YMCA programs for no compensation.

Authorization for EFT Transactions

I authorize the YMCA of Southern Nevada to initiate debit entries to my credit card account and authorize the credit card institution to deposit the same amount into the account of the YMCA of Southern Nevada.  This authority is to remain effective until written request for termination is received at least 30 days prior to termination.

Should any preauthorized EFT not be honored by my credit card institution when received by them, then it is understood  that the payment is to be made by me in the amount of said payment plus service charge.  It is further understood that if such payment is not honored by the credit card institution, then the YMCA of Southern Nevada, at its discretion, may resubmit the amount due for payment on a future date.

I understand that the YMCA of Southern Nevada Board of Directors may, at their discretion, adjust the monthly rate applicable to my category of membership.  I understand that I will receive at least four weeks notice prior to such change.
